<p>With the notoriety that HIV or Human Immunodeficiency virus has gained currently, HPV or Human papillomavirus is almost relegated to the sidelines. But its infection-causing potential has not grown any less. The problem is HPV is not a single virus; it has over 100 strains that can wreak havoc on the body. </p>
<h2>Understanding HPV</h2>
<p>HPV are DNA-based viruses that have the potential to infect the skin and mucous membranes of humans and a variety of animals. Youï¿½d be surprised to know that HPV is one of the most common causes of sexually transmitted infection in the world. There are more than 100 different types of strains, of which, about 30 are typically transmitted through sexual contact. As per the statistics offered by the Centers for Disease Control and Prevention, approximately 6.2 million new cases of HPV infections are reported every year. And at least 20 million people in the US are already infected! (1) </p>
<h2>HPV infections</h2>
<p>HPVs are associated with warts or papillomas. </p>
<p>These can be common warts or genital warts. The former is usually transmitted environmentally or acquired by casual skin-to-skin contact, whereas the latter is sexually transmitted. Letï¿½s see the HPV induced infections in some detail:</p>

<h3>a. Skin warts:</h3>
<p>     <strong>i.</strong> Common warts: Common warts are caused by HPV-1 and HPV-2 types. There usual place of occurrence is hands and feet, but they can also occur in other areas, such as the elbows or knees. These warts have a characteristic cauliflower-like surface and are typically slightly raised above the surrounding skin. The good news is these donï¿½t cause genital warts or cause cancer.</p>
<p>     <strong>ii.</strong> Plantar warts: Found on the soles of the feet, plantar warts closely resemble common warts.</p>
<p>     <strong>iii.</strong> Subungual or periungual warts: These warts can form subungually under the fingernail or around the fingernail or even periungually on the cuticle. Because of their location they pose difficulties in treatment than warts in other locations.</p>
<p>     <strong>iv.</strong> Flat warts: The scourge of children and teenagers, flat warts are most commonly found on the arms, face or forehead. Thankfully flat warts arenï¿½t associated with cancer in people with normal immune function.</p>
<h3>b. Genital or anal warts:</h3>
<p> Genital or anal warts are soft, moist and usually flesh colored. They sometimes appear in clusters that resemble cauliflower-like bumps; they can also be raised or flat, small or large. In women they appear on the vulva and cervix, and inside and surrounding the vagina and anus. In men, they can appear on the scrotum or penis. In some cases these warts occur on the thigh and groin (2). The HPV causing genital or anal warts may get transmitted to others through sexual contact, even if the person doesnï¿½t display overt symptoms of the infection. Remember, the HPV types that cause genital warts are not the same ones that cause cervical cancer (1).</p>
<h3>c. HPV-induced cancers: </h3>
<p>HPV-induced cancers, caused by a dozen or so ï¿½high riskï¿½ HPV types that include Types 16, 18, 31 and 45, can cause cervical cancer, anal cancer, vulvar cancer, head and neck cancers, and penile cancer (Parkin 2006). Women with no history of cervical cancer donï¿½t develop this cancer. Anal cancers usually occur in persons engaging in anal or oral sex. (1)</p>
<h3>d. Respiratory papillomatosis: </h3>
<p>This rare condition is caused by HPV Types 6 and 11. In this, warts form on the larynx or other areas of the respiratory tract (Wu et al., 2003; Sinal and Woods, 2005). (1)</p>
<p>HPV, if not treated in time, can cause endless suffering that may even lead to cancer. So, adopting safe sexual practices and keep a look out for anal or genital warts to catch this infection in its infancy.<br />

<h2>The low and high risk HPV</h2>
<p>Of the 100 different types of HPVs that exist, most are harmless. Some types of HPV only cause common skin warts, like those found on the hands and soles of the feet. These types of HPV, luckily, donï¿½t cause anal or genital warts. Only about 30 types have the ability to spread through sexual contact. </p>

<p>These are classified as either low risk or high risk. Low risk type cause genital warts, which may be single or multiple bumps that appear in the genital areas, including the vagina, cervix, vulva (area outside of the vagina), penis and rectum. High-risk types of HPV cause abnormal Pap smear results and could lead to cancers of the genital area. Many people infected with HPV display no symptoms at all. (1)</p>
<h2>Whoï¿½s at risk?</h2>
<p>If youï¿½ve had sex at an early age or with multiple partners without the use of barrier protection, youï¿½re at risk of getting infected with HPV, resulting in anal or genital warts. </p>
<h2>Risks of the infection</h2>
<p>Despite the extensive study of prevalent HPV infections, little is known about their incidence and natural history. Whatï¿½s surprising, findings from a variety of studies suggest that after infection, most individuals donï¿½t develop clinical signs or symptoms of this infection. However, all develop warts and a small percentage of infected individuals are likely to develop HPV-associated cancer (2).</p>
<p>Since, the various strains of HPV infect differently, some types cause cervical cancer, while other types are associated with vulvar cancer, anal cancer and cancer of the penis, a rare form of cancer. However, in women, most HPV infections donï¿½t progress to cervical cancer. But, still itï¿½s important to have regular pelvic exams and Pap smear tests done to detect them early, for timely treatment to be given, if necessary (1). Even HIV-positive men and women are at an increased risk of this infection. The risks for HPV-associated neoplasia and cancer, such as hypermethylation, leading to down-regulation of potential tumor suppressor genes, are also increased (3). </p>
<p>Anal and genital warts can cause a number of problems during pregnancy too, because these warts can multiply and become brittle, posing risk to the mother and baby. So, if necessary, itï¿½s best to discuss options for their removal with your physician. Warts, especially genital warts, can sometimes get larger during pregnancy. If they occur in the urinary tract, you may find it difficult to urinate; if theyï¿½re present in the vagina, they can make the vagina less elastic and cause obstruction during delivery. So, itï¿½s of utmost importance that they be removed to ensure a safe and healthy delivery of the newborn. (1) </p>
<p>Although rare, there have been cases where infants born to women with genital warts had developed warts in their throats. In medical parlance, this is termed as respiratory papillomatosis. For the child, this is a potentially life-threatening condition, requiring frequent laser surgery to prevent obstruction of the breathing passage. Research in interferon therapy with laser surgery shows promise in slowing the course of the disease. (1) </p>
<p>Anal warts hide a host of life threatening risks. So, itï¿½s always better to be safe than sorry by getting Pap smear and other tests done regularly, especially if youï¿½ve been a victim of anal or genital warts. </p>

<h2>Knowing anal wart-causing HPV</h2>
<p>There are more than 100 different types of HPV, of which, most are harmless. Itï¿½s these harmless ones that cause the external warts and, fortunately, donï¿½t lead to anal or genital warts. However, about 30 types (1) are spread through sexual contact and are classified as high risk, which can cause changes in the cells of the opening to the uterus or cervix, or anus. <h2>Mechanism of invasion</h2>

Once you come in bodily contact with an infected person, the viral particles penetrate the skin and mucosal surfaces through microscopic abrasions in the genital or anal area. When they enter the body, they donï¿½t cause infection right away. Theyï¿½ve a latency period, in which they generally lie dormant inside the tissues. It takes about a month to 6 months for them to get activated and get potent enough to cause warts on the skin. This period can be longer. However, if youï¿½ve sexual contact with a partner who already has these warts, youï¿½re most likely to develop them within 3 months. This can be after just a single episode.</p>
<p>Why this infection is so readily acquired from an infected partner is because itï¿½s highly contagious. You can get it during oral, vaginal or anal sex with an infected partner. You can also get it by skin-to-skin contact during vaginal, anal or oral sex with someone who is infected. In women, warts can not only occur in the anus, they can be present on the outside and inside of the vagina, on the cervix or around the anus. <h2>Signs of anal warts</h2>

They can display a variety of shapes and sizes ï¿½ large or small, singular or in clusters or groups. The appearance of anal warts can also differ from person to person. In some, they may appear as small, cauliflower-like clusters or like flat, white areas that resemble dry, flaky skin. Just have a look at the pictures to get the idea.</p>
<p>Anal warts can cause itching, burning, bleeding or oozing in the infected area (1). Pain is generally not associated with them, but occasionally minor bleeding can result from anal sex or bowel movements. If left untreated, anal warts can lead to bowel obstruction. Of course, these developments in a sensitive area are enough to cause alarm and warrant quick diagnoses.</p>
<h2>Characteristics of anal warts</h2>
<p>The warts occurring outside the rectum are called peri-anal, and those occurring inside and affecting the lower inch or two of the rectum are called intra-anal.</p>
<p>Being highly contagious, anal warts can be transmitted through direct contact, usually sexual in nature, with an infected person. If statistics are to be believed, two-thirds of people having sexual contact with infected individuals usually develop anal warts within 3 months. The good news is even though common warts appearing on hands and feet are caused by HPV, they canï¿½t cause anal warts. But the real bad news is anal warts can lead to anal cancer, especially among men who have sex with men (2).</p>
<p>Anal warts not only look disgusting, they can prove immensely traumatic both mentally and physically. So, what should you do? First and foremast pay close attention to your body. If anal warts appear, keep them under close observation, but not indefinitely. Get them checked by your personal physician, as early as possible, since a correct diagnosis will help in eradicating this infection permanently from the body. And last but not the least, if you are sexually active, adopt safe sex practices.</p>

This generally gets manifested as warts, but unfortunately, this infection can occur without warts too (3). </p>

<h2>Preventing anal warts</h2>
<p>Once you know the anal wart cause, its prevention simply boils down to safe sex practices. But here even condoms won&#8217;t help. A condom offers limited protection, as it doesnï¿½t cover all the skin, inviting infection. So, the best way is to avoid sex or sex play with those displaying visible warts (3). Another important point to note is not to share sex toys or anal lube with an infected partner. Even putting finger in the anus of an infected person or having his percum or semen on the fingers can transfer the infection to you (3). Although, soap and water doesn&#8217;t kill this virus, it&#8217;s still a good idea to shower well immediately after sex to wash the virus off the skin (3).</p>
<h2>Treating anal warts</h2>
<p>Anal wart treatment generally includes topical chemical solutions, topical anti-viral solutions, immunotherapy, cryotherapy (using liquid nitrogen), surgical removal and electro-surgery (2).</p>
<p>And the good news is you don&#8217;t have to be hospitalized for getting a cure for anal wart. Even the cautery and excision technique can be performed on an outpatient basis and the patient can go home after the procedure. Let&#8217;s see the various treatments (1):</p>
<p><strong>a.</strong> Medications: Very small warts, accessible around the anus, are treated with medications. However, your physician will need to exercise great care and precision in applying strong medicines directly to the surface of the warts, since injury to the normal skin surrounding the warts is possible. The treatment is a long drawn out one.</p>
<p><strong>b.</strong> Cauterization or surgical removal: As per dictionary.com, cautery is an agent or instrument used to destroy abnormal tissue by burning, searing or scarring, including caustic substances, electric currents, lasers and very hot or very cold instruments. Thus, electrical cautery, laser surgery and surgical removal are the general methods of removing the warts. These procedures are especially suited for warts inside the anal canal, which are usually not accessible for treatment by medications. The good news is once the warts are removed, they generally donï¿½t come back for months or years, even though the hpv may remain in the body (3).</p>
<p>So, if you&#8217;ve painless blemishes and bumps in your anal region, it&#8217;s time to visit your physician. Proper diagnosis and treatment will not only help you in curing this ailment, but in regaining your self esteem too.</p>
